When making a request for disclosure, etc., please fill in the required items on the Group's designated request form (see A below), enclose a copy of your identity verification document (see B below), and mail it to the address specified by the Group (see C below). In addition, a fee of 1,000 yen will be charged for each request for notification of the purpose of use and disclosure of personal information or records of provision to a third party. Please transfer the fee to the bank account specified by our company. Please note that the prescribed fee will also be charged in cases of non-disclosure and non-notification of the purpose of use.
